APMAN Revalidation / Registration form
As directed by the Board of Trustees (BOT), the AMPMAN membership revalidation
officially starts on the 14th March 2025.
Submission Process:
Members are required to submit the following via email: ampmanofficial@gmail.com and copy +2348064414164 via WhatsApp
1.Proof of Registration Payment or Official Evidence of Participation in past AMPMAN events.
2.Two (2) Photographs:
A. Clearer high-quality passport size photograph in corporate attire.
B. Full-size image photograph in corporate attire.
3.Personal Information:
•Full Name
•State Chapter Membership
•State of Origin
•Gender (M/F)
•Phone Number
All above information is for the AMPMAN IDENTIFICATION Card 🪪 and official upload on the AMPMAN website.
Once submitted, your profile will be featured on the AMPMAN website as a pending member until full revalidation is completed at our convention ground in Asaba, Delta State.
